Nearly 10 million people living with HIV were accessing antiretroviral
treatment last year, says a new United Nations report, which notes
that even more people can be reached with smart planning.
Last year's figure represents the biggest increase from one year to
another as numbers of people accessing antiretroviral treatment rose
by 1.6 million from 2011 to 2012, according to the report, released
today by the Joint UN Programme on HIV/AIDS (UNAIDS), the World Health
Organization (WHO) and the UN Children's Fund (UNICEF).
New guidelines from WHO, issued together with the report, give clear
recommendations that people living with HIV should start
antiretroviral therapy (ART) much earlier, and immediately in some
instances.
"These guidelines represent another leap ahead in a trend of
ever-higher goals and ever-greater achievements," said WHO
Director-General Margaret Chan.
"With nearly 10 million people now on antiretroviral therapy, we see
that such prospects -- unthinkable just a few years ago -- can now
fuel the momentum needed to push the HIV epidemic into irreversible
decline," Dr. Chan stated in a

release.
The guidelines recommend, among other measures, providing ART to all
children with HIV under the age of 5, all pregnant and breastfeeding
women with HIV, and to all HIV-positive partners where one partner in
the relationship is uninfected.
"Advances like these allow children and pregnant women to access
treatment earlier and more safely, and move us closer to our goal of
an AIDS-free generation," said UNICEF Executive Director Anthony Lake.
"Now, we must accelerate our efforts, investing in innovations that
allow us to test new born babies faster and giving them the
appropriate treatment so that they enjoy the best possible start in
life."
Under the new guidance, some 26 million people will now be eligible
for antiretroviral therapy, an additional 9.2 million from the
previous 2010 guidance.
"It is our moral and scientific obligation to reach as many people as
we can with antiretroviral therapy," UNAIDS Executive Director Michel
Sidibé said in a separate

release. "This is what we will continue to strive for and we believe
that we can significantly scale up access to treatment even within the
current financial envelope."
By making smart choices, UNAIDS estimates that treatment can be
further expanded within the existing resource needs of between $22-24
billion for 2015.
The agency estimates that cost savings could be achieved through three
main areas: a reduction in costs of medicines and medical supplies,
particularly as volumes increase; simplifying delivery systems; and
increasing efficiencies within the overall AIDS response.
In recent years, the price of medicines to prevent mother-to-child
transmission of HIV was reduced from $800 in 2011 to below $100 in
2013.
Through a more competitive bidding process, South Africa has reduced
the cost of procurement of antiretrovirals to the lowest price
anywhere in the world at $127 per person per year for the fixed dose
combination recommended in the new guidelines. This has resulted in a
53 per cent reduction in expenditure on antiretroviral treatment for
South Africa.
Additional savings are expected as methods of testing for HIV become
simpler and easier to administer (a fingerpick HIV test, for example,
can now give results in 30 minutes). Other efficiencies are being made
as more and more HIV services are being integrated into existing
structures such as antenatal clinics and tuberculosis facilities.
The challenge set by the new guidelines will encourage countries,
donors and partners in the AIDS response to strive for even greater
results. If the recommendations in the new guidelines are implemented
they would avert an estimated 13.5 million deaths and 19 million new
HIV infections by 2025.

Nearly 10 million people living with HIV were accessing antiretroviral
treatment last year, says a new United Nations report, which notes
that even more people can be reached with smart planning.
Last year's figure represents the biggest increase from one year to
another as numbers of people accessing antiretroviral treatment rose
by 1.6 million from 2011 to 2012, according to the report, released
today by the Joint UN Programme on HIV/AIDS (UNAIDS), the World Health
Organization (WHO) and the UN Children's Fund (UNICEF).
New guidelines from WHO, issued together with the report, give clear
recommendations that people living with HIV should start
antiretroviral therapy (ART) much earlier, and immediately in some
instances.
"These guidelines represent another leap ahead in a trend of
ever-higher goals and ever-greater achievements," said WHO
Director-General Margaret Chan.
"With nearly 10 million people now on antiretroviral therapy, we see
that such prospects -- unthinkable just a few years ago -- can now
fuel the momentum needed to push the HIV epidemic into irreversible
decline," Dr. Chan stated in a

The guidelines recommend, among other measures, providing ART to all
children with HIV under the age of 5, all pregnant and breastfeeding
women with HIV, and to all HIV-positive partners where one partner in
the relationship is uninfected.
"Advances like these allow children and pregnant women to access
treatment earlier and more safely, and move us closer to our goal of
an AIDS-free generation," said UNICEF Executive Director Anthony Lake.
"Now, we must accelerate our efforts, investing in innovations that
allow us to test new born babies faster and giving them the
appropriate treatment so that they enjoy the best possible start in
life."
Under the new guidance, some 26 million people will now be eligible
for antiretroviral therapy, an additional 9.2 million from the
previous 2010 guidance.
"It is our moral and scientific obligation to reach as many people as
we can with antiretroviral therapy," UNAIDS Executive Director Michel
Sidibé said in a separate

release. "This is what we will continue to strive for and we believe
that we can significantly scale up access to treatment even within the
current financial envelope."
By making smart choices, UNAIDS estimates that treatment can be
further expanded within the existing resource needs of between $22-24
billion for 2015.
The agency estimates that cost savings could be achieved through three
main areas: a reduction in costs of medicines and medical supplies,
particularly as volumes increase; simplifying delivery systems; and
increasing efficiencies within the overall AIDS response.
In recent years, the price of medicines to prevent mother-to-child
transmission of HIV was reduced from $800 in 2011 to below $100 in
2013.
Through a more competitive bidding process, South Africa has reduced
the cost of procurement of antiretrovirals to the lowest price
anywhere in the world at $127 per person per year for the fixed dose
combination recommended in the new guidelines. This has resulted in a
53 per cent reduction in expenditure on antiretroviral treatment for
South Africa.
Additional savings are expected as methods of testing for HIV become
simpler and easier to administer (a fingerpick HIV test, for example,
can now give results in 30 minutes). Other efficiencies are being made
as more and more HIV services are being integrated into existing
structures such as antenatal clinics and tuberculosis facilities.
The challenge set by the new guidelines will encourage countries,
donors and partners in the AIDS response to strive for even greater
results. If the recommendations in the new guidelines are implemented
they would avert an estimated 13.5 million deaths and 19 million new
HIV infections by 2025.

The purpose of today's hearing is to explore threats posed to
traditional marriage, historically understood as the union of one man
and one woman, by recent court decisions, including the United States
Supreme Court's Lawrence decision and the Massachusetts Supreme
Judicial Court's Goodridge decision. Despite the authority of
Congressto enact the Defense of Marriage Act under clear
constitutional provisions, which wasthe subject of our last hearing,
it is unfortunately becoming increasingly common to see once clearly
understood constitutional provisions wash away over time following a
slowly advancing tide of judicial precedence. For example, in 1965,
the Supreme Court in Griswold v. Connecticut discovered a
constitutional right to contraception rooted in the right to marital
privacy.By the time the Court decided Roe v. Wade in 1973, the right
to reproductive privacy was applied to abortion, wholly outside the
context of marriage. In 1986, the Court in Bowers v. Hardwick
refused to create a right of sexual privacy for same-sex couples, but
then in 2003, the Court reversed itself in Lawrence v. Texas. In
Lawrence, the Court claimed not to have gone so far as to establish a
right to same-sex marriage, but then the Massachusetts Supreme
Judicial Court prominently used the Lawrence decision just a few
months later to do just that. While the Massachusetts court
repeatedly cites in its decision the Massachusetts Constitution,
nowhere in the Goodridge decision did the court state precisely which
provisions of the Massachusetts Constitution had been violated by the
State's traditional marriage policy. Instead, the Massachusetts court
expansively cited Lawrence v. Texas as establishing a broad right of
personalautonomy, failing to acknowledge the statement in Lawrence
that ``the case does not involve whether the Government must give
formal recognition to any relationship that homosexual persons seek to
enter,'' and also failing to acknowledge any of the differences
between laws regulating private sexual behavior and laws establishing
public family relationships. The Massachusetts court in Goodridge
concluded there was ``no rational reason'' for restricting the
benefits of marriage to heterosexual couples. That court thus asserted
via what The Washington Post editorial page has called a judicial fiat
that the three reasons the State of Massachusetts gave for giving
preferred status to heterosexual marriage--promoting procreation,
encouraging the raising of children in two-parent biological families,
American society will come to formally recognize gay relationships as
a result of judicial fiats and we felt that the four-to-three majority
on the Massachusetts court had stretched to find a right to gay
marriage in that commonwealth's 224-year-old Constitution. When moral
certainty bleeds into judicial arrogance in this fashion, it deprives
the legislature of any ability to balance the interests of the
different constituencies who care passionately about the question.
Given the moral and religious anxiety many people feel on the subject
and the absence of clear constitutional mandates for gay marriage,
judges ought to be showing more respect for elected officials trying
to make this work through a political process,'' and again, that was

The 87-car Klahowya is back on schedule with the 6:30pm Vashon
departure to Fauntleroy. To preserve aging equipment on the
55-year-old vessel, we are operating it at speeds that do not allow it
to make up for any unforeseen delays on the
Fauntleroy/Vashon/Southworth route's tight schedule, and it is often
not able to keep the published sailing schedule. Updates will occur as
they become available.
